Registration with the practice

To register with the practice, please complete both the registration form and questionnaire.  You will need to make an appointment for a new patient check with one of our nursing assistants.  You will be asked for information on your medical history, medication, immunisations, lifestyle and general health and family history.  You will also have your blood pressure checked.

To register as a patient, please call at reception or click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us.  Ensure your address is within the practice boundary - if you are unsure, please check the practice boundary map or contact us.

You will be registered with Abbottswood Medical Centre but you can state which doctor you would normally prefer to see.  All patients, including children, are allocated a named, accountable GP, who is responsible for their overall care at the practice - this is referred to as your 'Usual GP'.  If you are not aware of your usual GP please do check with any member of staff at any time.

We encourage you to see you usual GP where possible, particularly for ongoing problems, so we can provide you with better continuity of care.  If he/she is fully booked, you can ask for an appointment with another doctor.  We will make reasonable efforts to accommodate requests to change Usual GP.

To enable us to commence your care whilst waiting for your NHS records to arrive, please complete a new patient questionnaire (call at reception or click on the link below to open the form) AND make an appointment to see a nursing assistant.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice along with proof of id as specified above.
