Proof of vaccination

We are unable to issue this certificate at the practice.

Proof of your vaccination status will be available on the NHSapp, (which is also valuable for ordering repeat prescriptions)
The NHSapp can be downloaded from https://apps.apple.com/gb/app/nhs-app/id1388411277 for iPhone or https://play.google.com/store/apps/details?id=com.nhs.online.nhsonline for Android. Once this is downloaded and you have uploaded your photo ID you will be able to view your GP health record, order a repeat prescription, check your Covid-19 vaccine record, share your Covid-19 status and manage your appointments. There are links to get advice about Coronavirus, you are able to search conditions and treatments plus contact NHS 111.
Alternatively you can call the NHS helpline on 119 (from 17 May) and ask for a letter to be posted to you. This must be at least 5 days after you’ve completed your course of the vaccine, the letter may to take up to 5 days to reach you.

Please don't telephone the Practice to enquire about getting your vaccination if you think you are eligible - our PCN will contact you with an invitation to book. Our telephone lines are extremely busy at present and we will need to ensure we are there for those patients who are acutely unwell.
