People with a learning disability may find some everyday activities more difficult to manage. These activities can include looking after their skin and teeth, understanding which foods are healthy, exercising regularly, and socialising with others. A learning disability can affect many areas of a person’s life, they may need additional support to help them stay healthy and well.

What happens when you get called in for a annual health check?

If you are 14 years old or older, you will be invited for a health check once every year. This will usually be around your birthday.

Your health check has two appointments:

  • One appointment with a nurse.

  • One appointment with a doctor (GP) who understands learning disabilities.

We will call you to book your appointment.
We will also send you a form to fill in. Please bring the form with you to your appointment.

You can bring someone with you, like a family member or support worker, if you would like.

At your appointment, the doctor will talk with you about your health.
They will give you a Health Action Plan to take home.
This plan will include important information about what was talked about, what you need to do next and how to keep fit and healthy.
