Regular annual reviews are an important part of managing long-term conditions and ensuring that your medication and treatment remain safe and effective. 

How We Invite You 

Our practice invites patients for reviews around their birth month through one or more of the following methods: 

• NHS App notification
• Text message (SMS)
• Letter
• Phone call 

If we do not receive a response after several attempts, we will follow our Non-Responder Policy for Long Term Conditions. This helps us make sure your care remains safe and that we take appropriate steps to contact you. 

Responding to an Invitation 

If you have received an invitation for your annual review, please respond promptly to arrange your appointment. Timely reviews help us keep your care up to date and ensure your treatment continues to be safe and meet your needs. 

Missed a Review or Changed Your Details? 

If you think you may have missed a review, or if your contact details have changed, please contact our practice reception team as soon as possible.
